Rhinoplasty in Miami, FL

For patients seeking natural rhinoplasty results in Miami, Ensō Plastic Surgery offers a level of expertise that sets it apart. Dr. Paul Durand is recognized as one of Miami's best rhinoplasty surgeons for his ability to deliver balanced, lasting results tailored to each patient's unique facial structure.

What is Rhinoplasty?

Rhinoplasty, often called a nose job, is a surgical procedure designed to reshape and refine the nose for cosmetic or functional reasons. It can improve the appearance of the nose, such as making it smaller, straighter, or more balanced, or be used to correct breathing problems like a deviated septum. Each procedure is equal parts art and science, with the ultimate goal of creating a profile that aligns with each patient’s unique facial structure.

Because of the skill needed, it’s essential to choose a surgeon who performs a high volume of these cases. As a specialist in both primary and revision surgery, Dr. Durand uses advanced methods to ensure each result is stable, functional, and natural looking.

What Can a Nose Job Correct?

Modern rhinoplasty has evolved significantly over the last decade. Today’s techniques are more precise and less invasive, allowing for better results with significantly less downtime. Rhinoplasty can address a broad spectrum of cosmetic and functional concerns, including:

  • A large or oversized nose
  • Hump on the bridge (dorsal hump)
  • A crooked or asymmetric nose
  • A bulbous or droopy tip
  • Overprojected or underprojected tip
  • Issues that impede normal breathing, such as a deviated septum
  • A wide or flat nasal bridge
  • Wide or flared nostrils

How is Rhinoplasty Performed?

Rhinoplasty is performed under general anesthesia and typically takes between 1-3 hours, depending on the complexity of the case.

Dr. Durand begins by making precise incisions, either entirely inside the nostrils (closed) or with a micro-incision across the columella (open), to access the bone and cartilage framework. And then following your surgical plan, reshapes the underlying structure. If you have breathing issues, he can also correct a deviated septum at this time.

In cases requiring extra support or augmentation, small cartilage grafts can be harvested from your own septum, ear, or rib to build an even more stable structure. Once all adjustments are complete, the skin is then re-draped over the new framework and the incisions are closed. A small external splint is then placed to protect your nose as it begins to heal.

Open vs Closed Rhinoplasty Procedures

Dr. Durand is an expert in both open and closed rhinoplasty, and will provide clear recommendations during your private consultation.

In a closed rhinoplasty, all incisions are made inside the nostrils, leaving no external scars. This is often ideal for patients focusing on bridge refinement or minor adjustments. Because the skin is less disturbed, many patients experience a faster recovery with less initial swelling.

An open rhinoplasty uses the same internal incisions plus one tiny, strategic micro-incision across the columella (the skin between the nostrils). This allows Dr. Durand to gently lift the nasal skin, providing the clear visibility needed for high-precision work. This is ideal for complex tip reshaping, revision surgery, or any case where achieving perfect symmetry and long-term stability is the priority.

While many patients worry about an external scar, this small incision is meticulously placed to follow the natural contours of your nose. Once healed, the mark is virtually invisible in almost every patient.

What Makes a Successful Rhinoplasty?

A successful rhinoplasty looks as if no work was completed. The nose should look entirely natural, perfectly blending in with your facial features. Achieving this requires a balance of three factors:

Individualized Planning

There is no universal "perfect" nose. Dr. Durand tailors every procedure to your specific facial anatomy and aesthetic goals, avoiding a one-size-fits-all approach.

Structural Integrity

The new nasal shape must maintain its integrity for decades. Techniques that prioritize long-term stability, ensuring the bridge and tip do not shift or lose definition, are extremely important.

Functional Excellence

Appearance should never come at the expense of your health. A successful rhinoplasty also means you breathe as well as (or better than) you did before surgery.

Typical Rhinoplasty Timeline

Consultation: A private consultation with Dr. Durand to discuss your goals and evaluate your nasal anatomy. A personalized surgical plan is developed based on facial structure and functional needs.

Pre-Op (7–10 Days Prior): Completion of lab work and medical clearance. You will receive instructions on medications and supplements to avoid to ensure a smooth recovery.

Day of Surgery: An outpatient procedure performed under general anesthesia. Surgery typically takes 1 to 3 hours, after which you can return home on the same day.

Early Recovery (Days 1–3): Swelling and bruising are at their highest. It’s important to focus on rest and keeping your head elevated to manage congestion.

Splint Removal (Days 5–7): Return to the Ensō office to have the external splint and any sutures removed. Most patients feel comfortable returning to work shortly after this visit.

Resuming Activity (Weeks 2–4): Visible bruising typically resolves. The majority of daily activities can be resumed, though strenuous exercise should be avoided.

Refinement (1–6 Months): Swelling gradually subsides, breathing improvements become more apparent, and the nose continues to soften into its new shape.

Final Result (Up to 12 Months): Once all residual swelling in the tip has resolved, your nose has settled into its new permanent, natural shape.

Rhinoplasty Recovery

All patient recovery is managed closely by Ensō and designed to get you back to your normal life as quickly as possible. Immediately following surgery, a small external splint is placed to protect your new profile, and is typically removed in our office just 5-7 days later.

Mild bruising and swelling are normal in the first week, however patients are often surprised by how manageable the process is. Discomfort is typically minimal and easily controlled with light medication. And within seven days, most patients can expect to return back to work, assuming it doesn’t involve any heavy physical exertion.

While the initial result is visible as soon as the splints come off, the final, polished results will fully mature within a year.

Techniques Available at Ensō Plastic Surgery

Ultrasonic Rhinoplasty

Ultrasonic rhinoplasty is a modern, minimally invasive surgical technique in which a piezotome device is used to reshape the nose. Unlike traditional rhinoplasty, which involves manually reshaping the nasal bones with tools, ultrasonic rhinoplasty uses ultrasonic waves to carefully modify the bones of the nose.

This technique allows for more accurate adjustments and reduces trauma to surrounding tissues, resulting in less bruising and swelling after the procedure. The surgeon controls the intensity of the ultrasonic waves, which allows for fine adjustments to the nasal shape, even in complex cases.

Recovery time is also typically faster than traditional rhinoplasty due to the gentler nature. Ultrasonic rhinoplasty is commonly used for cosmetic changes such as narrowing the nose, straightening a crooked nose or refining the nasal bridge.

Preservation Rhinoplasty

Preservation rhinoplasty aims to preserve more of the patient's own cartilage and bones, usually leading to less swelling and quicker recovery. Results also tend to be more natural-looking as some of the original features are preserved. Note that not everyone is a candidate for preservation rhinoplasty, and sometimes more than one technique needs to be used.

Revision Rhinoplasty

A revision rhinoplasty is a secondary surgery performed to correct or improve the results of a previous rhinoplasty. This procedure may be required if the initial surgery did not achieve the desired outcome or if the nose has shifted over time due to healing or scarring. Revision rhinoplasty is often more complex and challenging because the surgeon has to work with existing changes from the first surgery, where tissues can be more delicate.

Ethnic Rhinoplasty

Ethnic rhinoplasty is a specialized type of rhinoplasty that takes into account a person's ethnic background and unique facial features. The goal of the surgery is to enhance the appearance of the nose while respecting that individual’s ethnic identity. Different ethnic groups may have specific nasal characteristics, such as a wider or flatter nose, a different bridge shape, or a more prominent tip.

We perform a large volume of ethnic rhinoplasty, including:

  • Hispanic rhinoplasty
  • Middle eastern rhinoplasty
  • African-american rhinoplasty
  • Asian rhinoplasty

Male Rhinoplasty

Male rhinoplasty enhances the shape, size and overall appearance of the nose while maintaining strong, masculine facial features. The overall goal is to create a nose that complements the male face, rather than feminize it.

Teenage Rhinoplasty

Rhinoplasty is one of the few cosmetic surgeries that you can safely undergo as a teenager. However, it’s important that patients are at least 14 years old for females and 16 years old for males, as facial bones are not fully developed prior to these ages.
