What is a total capsulectomy and how does it differ from an en bloc capsulectomy?

A total capsulectomy means removal of the entire capsule (scar tissue) surrounding the implant. In most cases, the implant and capsule can be removed together in one intact piece, which is referred to as a total intact capsulectomy. This allows for complete
capsule removal while preserving surrounding healthy tissues.

An en bloc capsulectomy is a more specific oncologic surgical technique. It involves removal of the implant, capsule, and a margin of surrounding normal tissue together in one piece. This technique is primarily indicated when there is concern for implant-
associated cancer (such as BIA-ALCL), where obtaining negative margins is important for diagnosis and treatment. En bloc removal is not medically necessary for most patients.

A technique sometimes used in complex cases is a controlled total capsulectomy. In this approach, a small, deliberate incision is made in the capsule to remove the implant in a controlled manner without contaminating the surrounding breast pocket. The capsule is then carefully closed and removed entirely in one piece. This maneuver is often chosen when the capsule is densely adherent to critical structures such as nerves, blood vessels, or the rib cage—where patient safety takes priority over forcing an en bloc technique.
