The decision comes down to what your nose needs. Bridge work and straightforward reductions often lend themselves to a closed approach, while tip refinement, grafting, and correcting prior trauma usually call for the visibility an open approach provides.
Within the surgery itself, Dr. Durand works on the underlying bone and cartilage framework rather than just the surface of the nose. That might mean filing down a dorsal hump, narrowing the bridge, reshaping the tip with cartilage grafts taken from the septum, or opening up the airway if breathing has been an issue. The skin then redrapes over the new foundation, and a lightweight splint is placed to hold everything in position while it heals.
Recovery

After visible swelling resolves, a deeper layer of swelling continues to settle for months, especially around the tip. The shape you see at the splint removal is a strong preview, but the final product can take up to a full year to reveal itself. Dr. Durand: With male patients, the goal is almost always to refine without softening. That means preserving the height and straightness of the dorsum, keeping the tip projection where it belongs, and respecting the angles that read as masculine, particularly the nasolabial angle and the relationship between the nose and the brow. Over-rotating the tip or taking too much off the bridge are the two most common ways a male rhinoplasty starts to feminize a face, and avoiding that is something I'm thinking about from the moment we plan the surgery.
A lot of men come in with concerns from an old break or sports injury. How does that change things?
Dr. Durand: Old trauma is one of the most common reasons men end up in my office, and it adds a layer to the planning. A nose that was broken years ago often has both a cosmetic issue and a functional one. The bridge may be visibly crooked, but there's frequently an internal deviation or valve collapse contributing to breathing problems that the patient has lived with for so long they barely notice anymore.
When we address it, I'm working on the external shape and the internal airway at the same time. Most men are surprised by how much their breathing improves afterward, especially during exercise or sleep. It's often the part of the result they end up appreciating the most.
Some men worry rhinoplasty will make them look like they had work done. How do you avoid that?
Dr. Durand: I try to push patients toward a more conservative result than they sometimes ask for, particularly with tip work.
